Restore Physical Therapy and Pelvic Health chooses to be an out of network provider to ensure that you can receive all services with a Doctor of Physical Therapy and without the insurance companies dictating your care. All services are "fee for service." If you would like submit to your insurance company for possible out of network reimbursement, a superbill with all necessary information will be provided. Please contact your insurance company for more information about your out of network benefits and reimbursement.
